Temporal lobe epilepsy (TLE) is a subtype of focal epilepsy in which the medial temporal lobe (MTL) is the epileptogenic focus. Repetitive transcranial magnetic stimulation (rTMS) has been shown to improve the seizure control in patients with refractory TLE, where the MTL is the epileptogenic zone. The aim of this study was to assess the efficacy of rTMS in comparison to combined occipital and frontal stimulation in the control of TLE. Eighteen patients with refractory TLE and intractable seizures were evaluated. Patients were randomly allocated into two groups. Group 1 (n=9) received only the occipital stimulation with a figure-of-eight coil. Group 2 (n=9) received the combined rTMS (1 Hz, total 1200 pulses/day) with a figure-of-eight coil. All patients received the same medication protocol. Patients were evaluated during a baseline and after 3 months of treatment by a neurologist blind to the clinical outcome. The rTMS group showed a significant seizure reduction in both, frequency (85.9%, P=0.0007) and duration (89%, P=0.0004). There was also a significant reduction of seizure frequency and duration in the occipital stimulation group. The combination of rTMS and occipital stimulation showed an effect similar to that of rTMS alone, but with lower intensity. rTMS seems to be more effective in controlling TLE than occipital stimulation alone.
